The program that Forum Research Inc. (“Forum Research company”) provides on this site ("Site") is subject to the terms and conditions as set out below. Please note that the Forum Research company reserves the right to update and/or amend some or all of the Terms and Conditions at any time without prior notice. We will revise the "Last Updated" date at the top of the Terms and Conditions when these are changed.
By joining Forum Research’ online research panel (“Forum Fuzion panel”) and using this Site you acknowledge that you have read, understood and agreed to be bound by the most recently updated version of the Terms and Conditions and all documents incorporated by reference therein.

Forum Fuzion™ is the trademarked name for the online research panel ("Forum Fuzion panel") of the Forum Research company.
Conducting online surveys with members of the Forum Fuzion panel (“Forum Fuzion panel members”) enables the Forum Research company to provide their clients with anonymous customer feedback that helps them develop better products and services. Our research results do not identify individual customers.
Forum Fuzion panel members who participate in Forum research opportunities earn points they can use to obtain gift cards.
Gift cards
Forum Fuzion panel members who participate in the Forum research opportunities earn points that may be redeemed to obtain gift cards.
The availability of specific gift cards may vary from time to time and may include cards from companies such as Amazon, Cineplex, Indigo, iTunes, Tim Hortons or Starbucks.
Eligibility for participation
Participation in Forum Fuzion is limited to adults living in Canada or the United States of America. You must have reached or be older than the age of majority of the province, territory or state in which you live in order to join the Forum Fuzion panel.
If you have not reached the age of majority in the province or territory where you live, please do not register for the Forum Fuzion panel.
BY COMPLETING THE REGISTRATION PROCESS, YOU AGREE TO BE BOUND BY ALL OF THE TERMS AND CONDITIONS OF THIS AGREEMENT.
The Forum Research company may refuse its services, including participating in the Forum Fuzion panel, to anyone at any time, at its sole discretion.
Providing incorrect information about your age disqualifies you from participating in the Forum Fuzion panel and from receiving gift cards.
Additional conditions
a. Contacting Forum Fuzion panel members
As a member of the online panel of Forum Research, you agree that the Forum Research company will contact you to participate in research opportunities using the e-mail address you provide upon registration.
Panel members can change their email addresses at any time by logging into their panel account and make the necessary changes.
In the event that you communicate with Forum Research using text messages (SMS) please notify the Fuzion Panel Coordinator promptly if your telephone number changes (the Coordinator’s contact information appears at the beginning of these Terms and Conditions).
b. Participation in research opportunities
As a Forum Fuzion panel member the Forum Research company will from time to time invite you to participate in a research opportunity.
To participate in these research opportunities, you must click the link that is provided in your e-mail invitation, or cut and paste the link into your web browser. You will then be taken to the research opportunity page that provides detailed instructions.
Fuzion panel participants who complete the research opportunity earn points entitling them to choose from a variety of gift cards.
Attempting to complete the same research opportunity more than once so as to earn points will result in your disqualification as a Forum Fuzion panel member.
c. Additional research opportunities
You may from time to time be sent additional research opportunities by the Forum Research company on behalf of its clients, based on the demographic information you provide when you register to be a member of the Forum Fuzion online panel.
The number of research opportunities you will receive in a given month is based on the demographic profile required by the Forum Research company and/or its clients.
d. Gift cards
You will receive points for each research opportunity in which you participate as a member of the Forum Fuzion online panel. Your points can be redeemed for a variety of downloadable digital gift cards from different companies.
To receive your points, you must complete the research opportunity in its entirety in the time period specified. If you complete multiple copies of the same research opportunity, only the first response will be accepted; attempting to complete the same research opportunity more than once so as to earn points will result in your disqualification as a Forum Fuzion panel member.
e. Suspension
Without limiting any other steps it may take, the Forum Research company may at its sole discretion suspend or terminate your account and your access to the Site for any reason, including but not limited to the following:
(i) If it is apparent that you have not acted in good faith when participating in any research opportunity, such as by attempting to complete the research opportunity more than once for the purpose of earning points for gift cards
(ii) If it apparent that you have deliberately created additional accounts, exceeding the one account per member policy,
(iii) If you have engaged in illegal, fraudulent or dishonest activity, and/or
(iv) If your account remains inactive for twenty-four (24) consecutive months.
f. Withdrawal from the Panel
Closing Account
Forum Fuzion panel members may withdraw from the panel at any time by closing their account. Please send an email to panel@forumresearch.com to request an account closure.
Voluntary Participation and Withdrawal Rights
Participation in all research activities conducted through our panel is entirely voluntary. Panel members may choose to decline participation in any project without any obligation. Furthermore, participants reserve the right to withdraw from ongoing research at any stage, for any reason, without penalty or explanation. If a participant chooses to withdraw, any data provided up to that point will be securely deleted upon request, in accordance with applicable data protection laws and our privacy policy.
Breach of these terms and conditions
The Forum Research company may issue a warning to you, temporarily suspend your ability to participate or terminate your membership as a member of the Forum Fuzion panel if you breach any of these Terms and Conditions as updated from time to time. We may also suspend or terminate your membership if the Forum Research company are unable to verify or authenticate any information you provide when you register as a Forum Fuzion panel member.
Privacy
Information collected from Fuzion panel members is subject to the terms of the Forum Research Privacy Policy. This Privacy Policy is part of these Terms and Conditions.
Confidentiality requirements
By becoming a member of the Forum Fuzion panel, you agree to keep confidential and not disclose any information you obtain or learn from any survey, project, questions or questionnaire of the Forum Research company.
You agree you will use the information contained in any survey, project or questionnaire provided to you as a Forum Fuzion panel member strictly for the purpose of providing market research related data.
The kind of information you may learn in connection with a survey, project or questionnaire of the Forum Research company includes: trade secrets, proprietary information, media content, unique techniques, sketches, drawings, works of authorship, models, inventions, know-how, research, experimental work, development, design details and specifications, sales and merchandising plans and other information provided. Using this information for purposes other than for participation in the Forum Fuzion panel disqualifies you from participating in Forum Research and may subject you to legal action.
You agree to notify the Panel Coordinator promptly (using the contact information provided at the beginning of these Terms and Conditions) should you discover any loss or unauthorized disclosure of such information.
Disclaimer and limitation of liability
The performance of the Forum Fuzion Site(s) and all content contained on, downloaded or accessed from the Site(s) are provided to you on an "as is, where is, when available" basis. The Forum Research company disclaims all conditions, terms, representations and warranties, express or implied, statutory or otherwise, including but not limited to implied warranties of merchantability, merchantable quality, quality, fitness for a particular purpose, title and non-infringement, with respect to the Site(s) or any of the content made available on the Site(s).
The Forum Research company reserves the right, at its sole discretion, to modify, disable access to or discontinue, temporarily or permanently, any part or all of the Site(s) or any information or services offered thereon, including your password access (if any), without liability or notice to you.
The Forum Research company does not warrant that the Site(s) and the content are free from errors, computer viruses or other program limitations or that the Site(s) and the content will be accessible and perform in accordance with your expectations. You acknowledge that the Forum Research Company is and will not be liable for any defamatory, offensive or illegal conduct of third parties. The operation of the Site(s) may not be continuous or uninterrupted or secure. Security and privacy risks cannot be eliminated.
Agency
You and the Forum Research company are independent contractors, and no employee-employer, agency, partnership, joint venture, or franchisor-franchisee relationship is intended or created by this Agreement, its Terms and/or its Conditions.
Notices
Except as explicitly stated otherwise, any notices by panel members shall be given by email to panelsupport@forumresearch.com and any notices by Forum Research shall be given by email to the email address you provide to Forum Research during the registration process.
Notice shall be deemed to have been given within the twenty-four (24) hours following the sending of the email notice.
General application of laws
This Agreement shall be governed in all respects by and construed in accordance with the laws of the province of Ontario, Canada. The Forum Research company does not guarantee continuous or uninterrupted access to its services, and operation of the Site(s) may be adversely affected by factors beyond the control of the Forum Research company.
If any provision of this agreement is held to be invalid or unenforceable due to factors beyond the control of the Forum Research company, such provision shall be struck and the remaining provisions shall be enforced.
Headings are for reference purposes only and in no way define, limit, construe or describe the scope or extent of such section. The failure of the Forum Research company to act with respect to a breach by you or others does not waive its right to act with respect to subsequent or similar breaches.
Intellectual property rights
"Forum Research", “Forum Fuzion” and related trademarks and service marks appearing on the Site(s) are registered trademarks, trademarks or service marks of Forum Research. All other trademarks, product names and company names or logos cited herein in either this document, or on Forum Fuzion site are the property of their respective owners.
By agreeing to use this Site you hereby assign all of your rights, title and interests in and to all survey results, project results, questionnaire results, market research results, creative suggestions, ideas, concepts, or other information (collectively, the "Submissions") that you provide to the Forum Research company, so that Forum Research Inc. shall exclusively own all now known or hereafter existing rights to the Submissions of every kind and nature worldwide and shall be entitled to unrestricted use of the Submissions for any purpose whatsoever, commercial or otherwise, without compensation to you beyond that previously described. In addition, you waive all "moral rights" in such Submissions. You agree that you shall have no recourse against the Forum Research company for any alleged or actual infringement or misappropriation of any proprietary right in your Submissions.
Use of the site
The Site(s) and information and materials contained at, through or in this Site(s) including the text, graphics, sound files, video files, logos, images, arrangement and general design of the Site(s) (collectively, the "Content") is owned or licensed by the Forum Research company. You many not use this Site in any manner that could damage, disable, overburden or impair the Site(s) or interfere with any other party's use and enjoyment of the Site(s).
You agree that you will use the Site(s) for lawful purposes only and represent and warrant that you will not:
a. use the Site(s) to upload, post or otherwise transmit through or to the Site(s) any content that
(i) is unlawful, abusive, threatening, harmful, obscene, lewd, offensive, defamatory or otherwise objectionable,
(ii) might infringe the intellectual property rights, privacy rights, rights of publicity, or other proprietary rights of others, or
(iii) contains any viruses, trojan horses, time bombs, or any other harmful programs or elements;
b. disrupt, place unreasonable burdens or excessive loads on, interfere with or attempt to gain unauthorized access to any portion of the Site, the Forum Research company’s computer systems, servers or networks, or the passwords of other users;
c. provide false information about yourself (including without limitation your identity, location, age, or financial capacity) or impersonate any other person, or otherwise attempt to mislead others about your identity or the origin of any content, message or other communication;
d. collect information about other users without their consent or otherwise systematically extract data or data fields, including without limitation e-mail addresses; and/or
e. sell access to or the use of the Site(s), including any content contained on, downloaded or accessed from the Site.
By uploading, posting or otherwise transmitting any content through or to the Site you grant to the Forum Research company, its successors and assigns, a non-exclusive, world-wide, royalty free, perpetual, non-revocable licence to use the content for the purposes of its research.
Language
You agree that English is the language of the Site(s) and all transactions occurring in connection with the Site(s), and you agree to waive any right to use or rely upon any other language and translations. You agree to accept these Terms and Conditions as provided (in English).

Forum Research Privacy Policy
The Forum Research company is committed to safeguarding your privacy.
This Privacy Policy governs the collection, use and disclosure of personal information by Forum Research, and describes how your personal information will be treated when you participate in the Forum Fuzion panel.
Changes to the Privacy Policy
The Privacy Policy may change from time to time and when that happens the “last updated" date at the top of the privacy policy will be amended.
Material changes to the policy will be announced through a notice on the home page of Forum Research company (including the home pages of its affiliated companies).
What is Personal Information?
Personal information is any information that can identify you specifically either on its own or when combined with other information. This may include, but is not limited to, your name, gender, age, national origin, marital status, address, phone number, e-mail address and identification numbers such as loyalty program card numbers, etc.
How does the Forum Research company use your personal information?
The Forum Research company conducts various types of research with Forum Fuzion panel members to provide its clients with anonymous customer feedback to help them develop better products and services.
Forum Research only collects your personal information once you have registered as a panel member. It will not contact you directly unless you are a member.
The information you provide as a Forum Fuzion panel member will be stored in Canada.
Your personal information will only be used for the purposes of aggregate analysis: individuals are never identified in this analysis or in the reports of the Forum Research company.If these purposes change we will inform you of this change and ask for your permission to continue using this information.
You may at some point wish to obtain customer service in relation to a company’s product or service. In that case we will require your permission to provide your contact information to the company.
If you do not want the Forum Research company to use your information in its research, please do not register as a member of the Forum Fuzion or participate in its surveys, and please do not use this Site.

What personally identifiable information does Forum Research collect from me?
The Forum Research company collects information in several ways from different parts of our network of websites.
Some personal information is gathered when you register, as you will be asked for your name, email address, postal code, household information, and other demographic, behavioral and attitudinal information.
If you contact the Forum Research company we may keep a record of that correspondence.
Why do we collect personally identifiable information?
The Forum Research company collects information about you so that we can ensure that the questions we ask are relevant to you and your experience.
What do we do with your personal information?
When you register with the Forum Research company we analyse your answers to the Forum Fuzion panel survey questions in relation to the demographic information you have provided. For instance, we might study how younger and older people feel about certain purchases.
We do not include any information about you as a specific individual in the results that we provide to our clients.
With whom does Forum Research share my personal information?
The Forum Research company does not disclose any information that could identify you as a specific individual in our analyses for clients.
From time to time, however, Forum Fuzion panel members may want to be placed in contact with specific companies in order to obtain customer service. If that happens you must expressly ask the Forum Research company to provide your contact information to a specific company or companies.
Otherwise, we will only release your personal information when we believe in good faith that Canadian law requires it or when we have been asked to do so by a Canadian search warrant, court order or other legal process.
What are ‘cookies’ and how does Forum Research use them?
A cookie is a small amount of data that is sent to your browser from a web server and stored on your computer's hard drive. As part of offering and providing customizable and personalized services, the Forum Research company may use cookies to store and sometimes track information about you. Some areas on the Site where you are prompted to log-in or that are customizable may require that you accept cookies. This means you do not have to enter your login information many times in one day.
You have choices with respect to cookies. Changing your browser preferences gives you the choice to accept all cookies, to be notified when a cookie is set, or to reject all cookies. Please note that if you choose to reject all cookies you will be unable to use the Site or qualify for points to earn gift cards.
When and how does Forum Research obtain my consent?
When you register to be a Forum Fuzion panel member you are consenting to the collection by the Forum Research company of the information you provide, subject to this Privacy Policy. When you answer the questions you receive as a Forum Fuzion panel member you agree that we may use your personal information in the anonymized fashion described above under "Use of your personal information": that is, your specific identifiable information is not disclosed in our analysis or reports which are based on and present aggregated information.
You may also write the Forum Research company to ask that it provide your contact information to a company to obtain customer service.
What are my choices regarding collection, use, and distribution of my information?
If you do not want the Forum Research company to use the information you have provided to the Forum Fuzion panel for any one of the purposes outlined in the "Use of your personal information" paragraph above or as otherwise described in the Terms and Conditions, please do not participate in the panel.
